A government issued photo ID from the person responsible for the rental items.
At least two trade references who we can verify your credit worthiness.
100% deposit in advance until credit is established.
Certificate of Insurance:
Your COI should name Dallas Lighting and Grip, LLC as additionally insured and loss payee for the following coverages:
* General Liability with limits of $1mil/$2mil, naming Dallas Lighting and Grip, LLC as additional insured and loss payee on the policy.
* Equipment Rental Coverage for gear rented from DLG for at least total replacement values. Rented Miscellaneous Equipment coverage (or Inland Marine coverage that insures rented equipment) – Special Form, Replacement Cost valuation, includes coverage in transit and at unscheduled locations and that there is no exclusion for theft of equipment from an unattended vehicle.. It must indicate the limit for rented equipment and the deductible.
* Auto: Hired and non-owned Auto Physical Damage coverage with a limit of at least $125K naming Dallas Lighting and Grip, LLC as additional insured and loss payee. The deductible should be indicated. (Required for motor vehicle rentals and transportation)
